Vancomycin is an antibiotic given by drip for resistant infections. Levels guide the amount given and protect the kidneys.

Measured in hospital during treatment, often every one to three days.

• Kidney disease
• A sample taken too soon after the injection

• A larger body size or high kidney clearance
• A sample taken late

• The timing relative to the injection defines which range applies
• Pre-dose and post-dose ranges are entirely different

Just before the next dose is due, once at steady state.
