A past-infection pattern is the expected result in most adults.

What this test measures

Epstein-Barr virus causes glandular fever. The serology looks at three antibodies (VCA IgM, VCA IgG and EBNA) whose pattern shows whether infection is recent, past or absent.

Why doctors request it

Requested for a sore throat with swollen glands and tiredness, and to interpret unusual blood counts.

What commonly pushes it up

Recent infection (VCA IgM positive, EBNA negative)
Past infection (VCA IgG and EBNA positive), which is the pattern in most adults

What commonly pushes it down

A negative result is the usual finding

Everyday reasons for a value outside the range

Results can be equivocal and are then repeated
Recent vaccination or infection elsewhere can occasionally give a weak positive
